About

Optum San Diego serves as the single point of access for Assertive Community Treatment (ACT) and Strengths-Based Case Management (SBCM) referrals. Optum provides a standard routing system for the receipt and processing of referrals to the most appropriate program based on the information provided on the referral and additional collateral information available. Optum will collaborate with the case management programs throughout the referral process.

• Crisis Needs: If you have an immediate mental health crisis, please contact the Access and Crisis Line at 888-724-7240 or dial 911 for medical emergencies. 

• Housing: If you have needs for housing or placement, please call 2-1-1, visit https://211sandiego.org, or contact the Access and Crisis Line to obtain additional housing resources at 888-724-7240.

• Medical Needs: For all other medical needs, please contact your primary insurance carrier by dialing the phone number on the back of your insurance card.

Criteria Information:

How do I refer?

1.       The person being referred needs to meet criteria for the requested program or level of care in alignment with the established guidelines linked above.

2.       Ensure that the person being referred is aware of the referral and willing to participate in services prior to submitting a referral.

3.       Gather all necessary information to complete a detailed referral form to ensure the completion and appropriateness of the referral. This will support the SPOA team in determining the most appropriate referral pathway.

4.       Submit a referral form online by using the button at the bottom of this page. A confirmation email will be sent to the email provided in the form.

Please Note: The referral process typically takes up to 10 business days, this includes review of the submitted referral form, collaborating with the referring party to confirm any missing information, and review of supporting documentation to substantiate placement based on established criteria.
